1. Are any of the following categories of people regarded as having a cure? Please state, where possible, what illnesses they are likely to be able to cure.
(a) A woman whose maiden name is the same as her married name.
(b) The daughter of such a woman.
(c) The son of such a woman.
(d) A person whose father was dead when he was born.
(e) A seventh daughter.
(f) A seventh son.
(g) A seventh son of a seventh son.
2. Are there any other similar categories of people said to have a cure?
3. Do you know of any cures which are passed on from one person to another? What illnesses are they said to cure?
4. Is it usual for such cures to be passed on from man to woman or from woman to man?
5. Do you know the procedures used in any of the above cures? Please describe them as much as you know at the end of the questionnaire sheet, indicating where possible whether they belong to seventh sons, women with the same married and maiden names, etc., or whether they are passed on.
6. Do you know any old remedies which anyone can use, e.g. cures for warts, styes, colds, hives, etc.? Please describe any that you know at the end of the sheet.
8. Is there a faith healer in your area?
9. Is there anyone in your area who could be described as a herbalist?
10. Are there any holy wells nearby? Please state the location as closely as possible (e.g. townland).
11. Are there any other places (rocks, buildings, etc.) in your area where cures may be obtained? Please state the location as closely as possible.
12. How does one go about getting a cure at such places?
13. Do people from your area travel to any place of pilgrimage or anywhere outside your area (e.g. to Knock, Lourdes, etc.) for the purpose of being healed? Are there any regular or organised pilgrimages?
14. Do you know of anyone from your area who has travelled outside the area to find someone with a cure or a faith healer? To which places did they travel?
15. Is it usual for a person with a cure, or a faith healer, to expect payment from the sick person?
16. Have you heard it said that a healer may become sick because of his healing practice?
17. To the extent that cures work, why do you think they work?
18. Can you offer any explanation why seventh sons, women with the same married and maiden names, etc., should be said to have a cure?
